We provide comprehensive support throughout your VA disability claim process—including pre-screening, consultation, military record review, DBQ drafting, medical/psychiatric evaluations, and coordination with our legal partners for submission to the VA.

Yes. We offer a free 15-minute eligibility assessment conducted by our trained RN team. If eligibility is determined, you will be invited to schedule a 1-hour paid consultation with our Veteran Disability Coordinator for $150.

Any veteran, active-duty service member, or reservist with a history of illness, injury, or mental health conditions related to service can request a screening. Our team will determine if your case meets preliminary VA claim criteria.

The 1-hour session (45 min interview, 15 min documentation) gathers a detailed history of your condition, treatment, military service, symptoms, and impact on daily functioning. This session determines the best filing approach.

MOXO is the secure client portal used by Survivor Sources Inc. to manage all steps of your application—from intake to final submission. You will receive access to track progress, upload records, sign documents, and communicate with your care team.

Timelines vary by case complexity, number of conditions, and how quickly documentation is submitted. Most clients complete their application within 4–6 weeks of starting the full consultation.

Typically, we request your DD214, military service treatment records, any prior VA decisions, and recent medical records. We’ll guide you on obtaining these if needed.

Yes. Disability Benefits Questionnaires (DBQs) are medical forms the VA uses to evaluate your condition. We complete DBQs as part of your consultation process and evaluations.

Absolutely. For conditions requiring a direct medical opinion linking your condition to service, our providers can conduct evaluations and draft a Nexus Letter as part of the documentation packet.

Yes. Your Wellness Sources, our clinical partner, provides medical and psychiatric evaluations if needed to support your claim.

Yes. We specialize in multi-condition claims, mental health cases (e.g., PTSD, MST, depression), and claims that require detailed legal-medical coordination.

Yes. We assist with new claims, increased rating requests, secondary conditions, and appeals. We will review your VA decision and recommend next steps.

$0 for the 15-minute assessment; $150 for the 1-hour consultation. Fees for DBQs, evaluations, or Nexus Letters vary and may range up to $3,500 or 10% of first VA payment.

Payment plans are considered on a case-by-case basis, especially for clients with demonstrated financial hardship. We also collaborate with donors and foundations to assist some Veterans.

Yes. Once your claim is complete, our legal partner (Aegis Law Firm) will file it with the VA under a signed Power of Attorney (POA). You will also receive a copy of your submission.

Yes, to allow Aegis Law Firm to submit your claim and communicate with the VA on your behalf. This ensures your claim is handled professionally and legally.

You’ll receive updates via the VA and from our team. The MOXO portal allows you to monitor status updates and receive guidance while the VA processes your case (typically 4–6 months).
